Q: How soon can I receive a grant/discounted rates or become a member of the PWF?

A: Timing is all dependent on the service(s) an individual is in need of and also where the individual stands on our waiting list. Many times, grants can be issued within 30 days of acceptance to the PWF.

Q: How do I apply for the PWF?

A: To apply for a PWF grant and discounted services, simply visit fill our the application here: http://parkinsonswellnessfund.org/patient-application (It’s FREE!)

Q: Who is eligible for PWF?

A: Any individual living with Parkinson’s as well as caregivers.

Q: Who is eligible to become a PWF Network Provider?

A: PWF Network Providers qualify based on their credentials with in their area of practice and go through an individual and personal interview process.

Q: What are the advantages of becoming a member of the PWF?

A: Becoming a member of the PWF will gain you access to health providers in your area that have agreed to provide PWF member discounts.  Discounts vary by Professional. You will also be eligible for 100% funded grants.

Q: What types of health professionals are in the PWF Network?

A: Health Professionals can include, but are not limited to the following:

Acupuncturists, Audiologists, Assisted Living Facility Managers, Chiropractors,  Dance Class Instructors, Doctors of Medicine,  Educational Support trainers, Fitness Trainers, Gym Membership Managers, Home Modifications Experts, Home Care, In Home Respite Care, Nurse Practitioners, Nutrition Program Consultants, Occupational Therapists, Optometrists, Physical Therapists, Podiatrists, Physician Assistants, Psychologists, Service Dog Trainers, Speech Pathologists, In Home Respite Care Providers, Thai Chi Instructors, Transportation Assistance Providers and Yoga Instructors.

Q: Have these Health Professionals been screened by PWF?

A: All PWF Network providers have provided us with proof of professional, certification and/ or license requirements according to state and federal laws and regulations.

Q: What will this PWF Network membership cost me?

A: FREE. Yes, some things in life, are free 🙂 There is absolutely no cost to Parkinson‘s families. This is a free service open to all members of the PWF who complete the application.

Q: What are the Play for Parkinson's programs for the PWF?

A: Play for Parkinson‘s exists to increase awareness regarding the affects of Parkinson’s and aid in breaking down the barriers of knowledge.  These programs range from tournaments to leagues to one-day events and spans across the tennis, soccer, golf and running communities.
